Arizona Medical Marijuana Card Renewal and Application Process
It’s Easier Than You Think
Qualifying and applying for a medical Marijuana card in Arizona can be challenging if you aren’t familiar with the process. That’s why we’ve broken the task up into four easy-to-follow steps.
Step 1: Check Your Qualifications
Arizona residents must go through a medical evaluation and complete an application before they are allowed to carry a medical Marijuana card. Licensed medical Marijuana dispensaries can only prescribe treatment for a short list of qualifying medical conditions. This list includes:
- Alzheimer’s Disease
- Chronic Pain
- Chron’s Disease
- Seizures
- Heptitis C
- PTSD
- Cancer
- Glaucoma
- AIDS/HIV
- Nausea
If you are suffering from one or more of these conditions, then you may qualify for medical Marijuana treatment. Follow the rest of these steps to get your card as quickly as possible.
Step 2: Find a Doctor
Locate a medical Marijuana doctor in your area.
Set an appointment and the doctor will evaluate you. Once he or she determines that you qualify for medical Marijuana treatment, you will receive a Physician Certification form. Hold onto this form, as you will submit it when you officially apply for your card.
Step 3: Finish Your Paperwork
In order to receive your medical Marijuana card, you will be required to submit your Physician Certification form along with the Qualifying Patient Application to the Arizona Department of Health Services.

Step 4: Find a Dispensary Near You!
The final step in the process is finding a conveniently located medical Marijuana dispensary. There are many to choose from across the valley.
